All information for current and incoming band members and forms to be completed for the 2020-2021 school year are located below.
PLEASE NOTE THE FOLLOWING:
- All students are required to complete the drug testing consent form.
- All students are required to complete and have NOTARIZED the medical release/field trip permission form (PDF type in below)
- All students/parents are expected to SIGN and turn in the final page of the Social Media Guidelines.
- All forms may be submitted electronically by emailing them to (TBD)
- All forms are due no later than July 10th; earlier is ok!

- Complete the fillable portions of the form.
- Save the form to your computer or desktop and add the student's first and last name to the file name. Do not delete the name of the file already provided.
- Attach any required signatures or print, sign and scan the form
